Inflammation-related variables are coded (as per NHANES 2005–2008) as follows: SMQ040 (current smoking status), MCQ010 (medical diagnosis of asthma), MCQ220 (medical diagnosis of cancer), MCQ160A (medical diagnosis of arthritis), Any heart disease (medical diagnosis of one or more of heart attack, congestive heart failure or coronary heart disease)
